Responsible for providing quality patient care at the Outpatient Mental Health Clinic. Consistently ensures patient dignity and respects patient values and is generally accepted by the professional community as safe and effective treatment. Psychiatric Nurse Practitioner provides Psychiatric assessments, consultation/liaison evaluations, medication management and education, recommendations and referral for treatment, individual and family treatment, and crisis intervention counseling. Integral member of interdisciplinary team and ensures patient receives continuity of care. Expanded scope of Psychiatric Nurse Practitioner's responsibilities as changes in healthcare landscape/regulations and operations demand, including, but not limited to on call responsibilities, performance Improvement activities, PCMH model of care, and CDI.
-Conducts initial and follow up evaluations on patients and develops appropriate treatment plans.
-Communicates effectively with families and agencies, conveying a positive, professional image.
-Demonstrates the ability to communicate needs of patients with treatment team.
-Provides psychopharmacology to patients and education.
-Provides crisis intervention counseling to patients.
-Attends staff and Treatment Team meetings as requested.
-Provides injections for patients as necessary following Policy and Procedure, as indicated.
-Documents patient information in the electronic chart in accordance with all regulatory agencies,
including OMH, OASAS, DOH, CMS and DNV. Completes all necessary paperwork, signatures and
progress notes appropriate to the clinical situation, and in a legible manner.
-Supports performance Improvement initiatives as they arise to ensure quality patient care.
-Follows all Program and Hospital policies and procedures.
-Performs other duties, within Psychiatric NP's scope of practice, as requested by Chair of Psychiatry
-Supports and complies with all Medical Center and departmental policies and procedures.
-Attends all mandatory in-services, continuing education, annual health requirements, etc.
Licensed to practice as a Psychiatric Nurse Practitioner within the scope of practice as defined in NY State.
Member of the hospital medical staff.
Experience with psychiatric patients, and patients with co-occurring disorder; well-developed skills in integrating individual treatment approaches into treatment milieu, and good working relationships with other physicians and staff.
