The primary care Physician is responsible for examining and treating all members of a family, regardless of age or sex. Ensures proper injury care and disease prevention, diagnosis, treatment, and recovery. Promotes wellness and a healthy lifestyle to the community population. Prescribes and administers medications, performs routine vaccinations, and provides advice regarding personal health and hygiene. Requests necessary tests and follow-up visits and refers patients to specialists, as indicated. Collaborates with internal and external providers to provide quality care for their patients. Participates in community health and wellness events as requested. Promotes the Patient Centered Medical Home (PCMH) model of care for patients. Complies with all local, NY State and federal laws, regulatory agency standards and organizational policies and procedures (e.g. CMS, DNV, NCQA, HIPAA, DOH, OSHA, etc. Participates in Performance & Quality Improvement activities and committees to further improve quality of patient outcomes for the organization.
We are looking for an experienced, and compassionate Primary Care Physician to provide care for our patients in a community setting, located in Bellport, NY.
Requires a degree in medicine from an accredited school and is licensed to practice in New York State, as an MD or a DO.
Experience or familiarity with NCQA Patient Centered Medical Home standards preferred. Excellent communication and interpersonal skills, with attention to detail required. Experience with an Outpatient EMR preferred.
