The Director is accountable for exploring and executing integration of operations where cost efficiencies and improved patient care can be accomplished. Coaches and develops members of the care management team and builds the internal resources to promote innovation in care management practices. Develops and implements operations to facilitate data driven and outcome oriented case management services. Responsible for the strategic management of the operations as it pertains to case management, utilization review, discharge planning, social services, and annual plans and budgets. The incumbent in this position is responsible for the design and implementation of a case management function that effectively balances quality, satisfaction and cost effectiveness, and improvement of clinical and quality indicators.
· The analytical skills necessary to cost-effectively direct day-to-day operations, ensure efficient and appropriate scheduling of personnel, respond to any complex patient or personnel problem, create and manage a budget, and plan for the future requirements and responsibilities of the department.
· The interpersonal skill necessary to provide effective leadership and to develop and maintain cooperative, professional working relationships with the other Hospital departments and Medical Staff.
· The ability to communicate effectively with patients, their family members, physicians, nursing, and other Hospital personnel when exchanging patient status, care, and treatment information.
· The ability to recognize and address the psychosocial needs of patients and their families, and the ability to problem solve to attend to special needs relating to the patient's age and diagnosis.
