The Director of The Department of Care Management is responsible to provide the oversight, administrative direction that requires clinical experience and an advanced degree. Individuals in this role must also possess comprehensive knowledge of the healthcare industry, familiarity with government regulations on healthcare plans, strategic planning skills and an understanding of the operations of a healthcare organization.

The Director is accountable for exploring and executing integration of operations where cost efficiencies and improved patient care can be accomplished.  Coaches and develops members of the care management team and builds the internal resources to promote innovation in care management practices.  Develops and implements operations to facilitate data driven and outcome oriented case management services. Responsible for the strategic management of the operations as it pertains to case management, utilization review, discharge planning, social services, and annual plans and budgets. The incumbent in this position is responsible for the design and implementation of a case management function that effectively balances quality, satisfaction and cost effectiveness, and improvement of clinical and quality indicators.


  • LCSW or NYS RN License.       Master’s Degree and 10-15 years progressive experience in healthcare preferably a management role. Solid knowledge of transition planning, utilization review, social services, and annual plans and budgets. A Certified Case Management designation is preferred.


·         The analytical skills necessary to cost-effectively direct day-to-day operations, ensure efficient and appropriate scheduling of personnel, respond to any complex patient or personnel problem, create and manage a budget, and plan for the future requirements and responsibilities of the department.

·         The interpersonal skill necessary to provide effective leadership and to develop and maintain cooperative, professional working relationships with the other Hospital departments and Medical Staff.

·         The ability to communicate effectively with patients, their family members, physicians, nursing, and other Hospital personnel when exchanging patient status, care, and treatment information.

  • Strong communication skills necessary to lead or participate in meetings and in service education.

·         The ability to recognize and address the psychosocial needs of patients and their families, and the ability to problem solve to attend to special needs relating to the patient's age and diagnosis.
