This position assesses and responds to the needs of the terminally ill patient and/or their families and significant others and develops a plan of care to meet these needs; ensures appropriate clinical intervention, communicates pertinent information to the physician; offers support to the patient and family. This may include interaction/guidance over the phone and also home visits to provide pain and symptom management and pronouncement of death. The plan of care may be carried out in the home, residential health care facility or hospital. The major focus of the plan of care will be symptom management and pain control.
Applicant must possess a current New York State license to practice as a Registered Professional Nurse. BSN preferred. Applicant must possess: physical assessment skills; recent Community Health, Hospice, Pallative or Medical-Surgical Nursing experience; the ability to work with other groups to coordinate care; current BLS; current NYS driver’s license; the ability to read, understand, and interpret a map
