  • Overview

    Responsible for the organizing and evaluation of the planning and administering of nursing care for all patients in the Home Care program. In doing so, assigns duties to Community Health Nurses based on patients’ needs, staffing availability and so forth.  Directs preparation and maintenance of patients’ clinical records.



      • Ensures the provision of effective, efficient and safe home nursing in accordance with established regulatory and accrediting body standards, and agency policies and procedures.
      • Assures that all clinical personnel are competent in the field initially and ongoing by assessing skills on visits.
      • Oversees clinical staff in planning and implementing the delivery of patient care according to identified patient needs, and assists in communicating and coordinating patient care.
      • Utilizes knowledge and skills of department resource personnel in planning and implementing care and resolving complex problems.
      • Interviews, hires, orients, trains and evaluates the performance of ,and when necessary recommends discipline and discharge of subordinates.
      • Oversees contract services(i.e.therapists,aides and so forth) to ensure proper documentation.
      • Communicates as necessary with other departmental personnel to coordinate efforts, resolve operational problems and improve the quality of care to patients.
      • Schedules supervisory visits of clinical personnel in the field annually and as needed.
      • Performs other duties and responsibilities as required by a CHN supervisor which include but are not limited to home visits, on-call backups as necessary.
      • Conducts team meetings between the monthly staff meetings to keep lines of communication open.
      • Reviews and is responsible for seeing that all patient records are maintained as required by the Department of Health. Reviews a random 10% of nursing caseload monthly for documentation compliance and appropriate medical follow up..
      • Oversees that clinical staff performs a 10% active medical record review on an ongoing basis.
      • Initial records are reviewed daily and released to facilitate coordination and reimbursement. All monthly QAMs are released by the time the month closes.  
      • At all times is aware of the need for the confidentiality of the medical record and facilitates the same by observing use of the PC and notebook functions and advising corrective action as needed.


      • In collaboration with management staff implements and participates in the Home Health Agency performance improvement program.
      • Actively participates on agency committees.

      • Administers departmental policies and procedures in collaboration with multidisciplinary and management staff.
      • Maintains knowledge of current trends and developments in the field by reading appropriate books, journals and other literature and attending related seminars, conferences and the like.
      • Reviews documentation on admission to home care and on-going to ensure appropriate patient care is being rendered.
      • Attends and participates in all staff meetings.
      • Participates in the flu vaccine clinic by administering injections organizing and monitoring the clinic and correcting operational problems.
      • Administers PPD’s to Swezey Pavilion employees as requested.
      • Meets the monthly and annual budget requirements for Start of Care visits and overall operating expenditures.
      • Creates a customer (internal and external) focused environment by driving changes in process and procedure from the perspective of what is important to the customer.     
      • Plans, identifies, develops, prioritizes, and assigns departmental tasks/projects. Redirects day-to-day activities based on departmental/organizational needs.     
      • Involved in some or all aspects of selecting, hiring and orienting new hires to the department.     
      • Ensures that all staff members have a performance planning discussion outlining job functions and performance expectations. Gives employees feedback on how they are doing their jobs and recognizes employees for good performance.
      • Works effectively in a variety of situations and recognizes the need for change.
      • Has the ability to organize work and use time effectively to complete deadlines.
      • Identifies and ensures that staff is competent and has adequate training to perform job requirements. Acts as a professional and educational resource for staff.
      • Assures that staffing levels are maintained, schedules are coordinated, overtime is monitored, and assignments are managed to ensure effective departmental workflow.
      • Rotates responsibility for 24-hour call including visits and weekend/holiday supervisory coverage when needed.
      • Creates a climate of openness and trust by encouraging an open exchange of ideas and different points of view and listens carefully to what others have to say.
      • Works effectively with people in other departments, and encourages people to share ideas and information with others.
      • Ensure that staff comply with governmental, regulatory, and JCAHO standards.
      • Actively participates in the performance improvement process.
      • Maintains current and ongoing knowledge in the field.
      • Develops and implements plans for self-improvement.
      • Demonstrates the ability to enter patients into the Quick Entry system as to not delay admissions into the agency.
      • Accepts other duties as assigned by the DPS/Administrator
      • Informs the DPS when operational/patient/problems occur.


    -          Professional knowledge of nursing theory, practice and administration.               

        Current New York State License as a Registered Nurse.

        Current CPR/BLS

        A Bachelors of Science Degree in Nursing and two years experience in home care or the following combination; four years’ experience in home care and six credit hours or the equivalent of education/training in public health and principles of management.

        Interpersonal skills.

        Proficiency in Current CMS regulation
